Objective: The SMARTDIABETES project aims to create innovative,
value-based diabetes management models through digital tools, particularly
focusing on the elderly population. It seeks to enhance patient empowerment,
facilitate healthcare provider decision-making, and reduce healthcare costs by
integrating data-driven approaches and AI technology into existing healthcare
systems.
Key Activities:
· Implementation of SMARTDIABETES
in clinical sites across Europe.
· Development of an operational
model to support healthcare providers and payors.
· Creation of value-driven
reimbursement strategies to promote scalability.
· Training programs and patient
onboarding for efficient use of the digital tools.
Expected Results:
· Improved diabetes self-management
for patients.
· Enhanced quality of life for
patients over 65.
· A comprehensive reimbursement
model to ensure long-term sustainability.
